Student Verification
The Office of the Registrar can provide proof of enrollment at SIUE for reasons including but not limited to, health insurance, loan deferment, or tax purposes.
Please print, complete, and sign (hand and digital signatures accepted) the Student Verification Form found on our student forms page.

Degree Verification
Southern Illinois University Edwardsville has authorized the National Student Clearinghouse to provide degree verifications.

Instructions for Veterans Certification
To request enrollment certification for the Department of Veterans' Affairs educational benefits, you should complete the GI Bill Certification Request Form. Printable or electronic versions are available.
Print, sign, and mail the form to:

Please note, you should submit a GI Bill Certification Request Form for each term you wish to have your enrollment certified to the Department of Veterans' Affairs.
You can submit the GI Bill Certification Request Form for a term as soon as you register for classes. If you do not want your enrollment certified to the Department of Veterans' Affairs for a term, do not submit a GI Bill Certification Request Form for that term.
Course adds, drops, withdrawals, cancellations and changes of program made after you have submitted a GI Bill Certification Request Form for the term should be reported as soon as possible to the Veterans Certification Office (located in Rendleman Hall, Room 1207) You may submit either a revised paper form or the online form.
