Accepting The Offer of Admission
To accept the offer of admission, students must take the following first steps.
- $300 enrollment deposit - Students must pay a one-time enrollment deposit of $300 (applied to tuition expenses) check or money order payable to Southern Illinois University. Detailed instructions will be provided with the offer of admission. This deposit is non-refundable.
- Certification of Disciplinary History – Admitted students are required to give SIU/SDM permission to request and obtain disciplinary history from each college/university. This will be included with acceptance letter packet email.
- Criminal background check - Shortly after being offered admission, students will receive an email from Certiphi Screening Inc. with instructions on how to initiate the criminal background check process. All admitted students will be required to complete this background check.
Matriculation Documents
Admitted students must complete and turn in the following documents to finalize admission. Detailed instructions and links to the forms will be provided upon receipt of the enrollment deposit.
Class Survey
Incoming students are required to fill out a secure online survey through Qualtrics to provide the School of Dental Medicine with additional information.
Health Immunization Requirements
SIU School of Dental Medicine has a specific list of required immunizations with the immunization form (completed by physician).

Please submit the form prior to the first day of fall classes. Please note that a $25.00 fine will be levied on any student for failure to turn in a complete immunization record.
Life Support Training
As you will be involved in the care of patients as early as your first semester of the curriculum, you will also be required to provide proof of “Basic Life Support of Healthcare Providers” training.
You should schedule your BLS course no earlier the month of June in order to maintain a current training status through your scheduled recertification date which will be in June of the summer between your 2nd and 3rd academic years.

Transcripts
To finalize admission, students must have their official transcripts, with final grades and degrees, sent directly from all colleges or universities attended to the Office of Admissions. Detailed instructions on what types of transcripts to send and when will be provided.

Health Insurance
Health Insurance is not mandatory but is available to dental students through SIUE. See Academic Health Plans for more information.

SIU School of Dental Medicine is approved by the State Approving Agency for Veterans Education. Veterans must meet specific academic progress requirements to remain eligible for VA benefits. VA benefits are determined by the veteran’s length of active duty in service, number of dependents, enrollment status, “kickers” awarded by the branch of military service in which the veteran served, and other factors. After registering each term, students receiving VA benefits should provide written request to the Office of Admissions & Student Services to process your benefits. This request can be made by email to [email protected] for convenience. While a Veteran’s payment from the United States Department of Veterans Affairs is pending to SIU School of Dental Medicine, SIU SDM will not prevent enrollment; assess a late penalty fee to the student account; require the student to secure alternative or additional funding; deny student’s access to any resource (classes, lab, or clinic) available to other students who have satisfied their tuition and fee bills to the institution. Any change in enrollment after registration should be reported to the Office of Admissions & Student Services as soon as possible.
